Complaint form

If you are not satisfied with the services provided by a physician or believe that they did not fulfil their professional obligations, you may choose to inform the Collège of the situation by submitting a request for an inquiry. You can:

You will find information about the inquiry process in the leaflet How to file a complaint against a physician and and in this short video clip (in French only):

Please note that Dr. Steven Lapointe no longer holds the position of Director of Inquiries at the Collège des médecins du Québec. You can consult the section on the Collège's permanent staff to learn more about the directors and staff members. We are continuing to show the video because the information is still valid.

To claim damages and interest for harm you believe you have suffered, you must apply to the civil courts. Proceedings involving the Inquiries Division are not aimed at granting compensation.

Finally, anyone who wishes to report a person who is engaged in the illegal practice of medical activities can do so by contacting the Inquiries Division.

Inquiry requests can be sent by email or by mail. Please make sure you indicate your contact information clearly so that we can reach you. Our contact information is provided below. The Collège would also like to remind you that inquiry requests must be made in writing.
