Certificate of professional conduct and Certificate of registration on the educational register

A certificate of professional conduct is a record of the professional career of a member or ex-member of the Collège. 

A certificate of registration on the educational register is a record of the history of a student, resident or fellow who is or was enrolled in a medical program in Quebec.

The certificate of professional conduct issued by the Collège is based on the information in the professional file.

As a medical professional (resident, fellow, physician, etc.), you may sometimes be required to provide the result of the verification of your professional file to organizations or you may wish to find out the result for yourself. If so, you will have to consent to the release of information by completing the form provided for this purpose through the Collège’s Online services and pay the required fee.

NOTE - The following documents that date back less than 10 years are enclosed in full with any certificate of professional conduct:

  • letters sent to a physician by the Syndic or an Assistant Syndic following an inquiry and entered in the professional file when a finding of professional misconduct is made;
  • remarks and recommendations made to a physician by the Professional Inspection Committee and entered in the professional file.

At the Syndic’s discretion, letters following an inquiry that date back more than 10 years may be enclosed with the certificate of professional conduct. If documents enclosed with your certificate are written only in French, you can have them translated into English through the Collège for a fee. We will contact you if necessary.

Processing of the applications

Applications are processed by the Collège on a first-come, first-served basis. It usually takes around 10 business days for a certificate to be issued. However, if the documents enclosed with the certificate have to be translated into English, processing may take up to 20 business days.

If a form such as a Verification of licensure form must be completed, please send it to us by fax to 514 933-5167 or by email to ccp@cmq.org. Please note that you do not have to send the Collège the Consent to Release Information or Confirmation of Standing by Medical Licensing Authority forms from one of Canada’s Colleges of Physicians and Surgeons (CPS). The Collège will send the CPS in question all the information required to process your application.
 
The certificate is sent by regular mail to the organization concerned by the application or electronically through the Collège’s secure messaging system if the organization has joined the system. Alternatively, the document may be sent in a sealed envelope to your personal address in order for you to mail it to the organization in question. Only one original copy of the certificate is sent to the applicant or the organization.

Once the certificate is ready, you will receive an email confirming that the original certificate has been sent to the organization indicated in the application and that a scanned copy of the certificate has been sent to your personal page.
