Certificate of professional conduct


A certificate of professional conduct is a record of the professional career of a member or ex-member of the Collège des médecins du Québec (CMQ). A certificate of registration on the educational register is a record of the history of a student, resident or fellow who is or was enrolled in a medical program in Québec.

The certificate issued by the CMQ is based on the information in the person’s professional file.

As a medical professional (physician, resident, fellow, etc.), sometimes you have to provide the result of the verification of your professional record to organizations or to acquaint with it yourself. You will then have to consent to the disclosure of information, by completing the form provided for this purpose through the Online Services of the CMQ, and pay the required fees.

NOTE - the following documents that date back less than 10 years are included in full, with each certificate of professional conduct:

  • the letters sent to a physician by the Syndic or an Assistant Syndic following an inquiry and entered in the professional record when a finding of professional misconduct is made;

  • the remarks and recommendations made to a physician by the Professional Inspection Committee and entered in the professional record.

At the Syndic and the Responsible of the professional inspection's discretion, letters following an inquiry or remarks and recommendations of the Professional Inspection Committee that date back more than 10 years may be included with the certificate of professional conduct. When attachments to your certificate are written only in French, it is possible to have them translated into English through the CMQ for additional translation fees. We will communicate with you, if applicable.

Processing of the applications

Applications will be processed on a first-come first-served basis. It usually takes around 10 business days for a certificate to be issued. However, if the documents attached to the certificate have to be translated into English, processing may take up to 20 business days.

If you are required to complete a form such as the “Verification of licensure” form, please send it to us by fax at 514-933-5167 or by email to: ccp@cmq.org. Please note that you are not required to send the CMQ the form “Consent to Release Information” from one of the College of Physicians and Surgeons (CPS) of Canada. The CMQ will send the CPS concerned all the information required to process your application.

The certificate is forwarded by mail to the addressee indicated in the request or electronically through the CMQ’s secured messaging system, if the organization has joined. The document can also be transmitted in a sealed envelope to the applicant who will then transmit it to the addressee. Only one original copy of the certificate can be sent to an applicant or organization.

The applicant will receive an email once the certificate is issued. Then, a scanned copy of the issued certificate will be available in his/her personal page.
