If you would like to inform the CMQ that someone is engaging in one or more medical activities illegally or is usurping the title of physician, you can fill out the “Report concerning the illegal practice of medicine” form.

Report concerning a physician

If you are not satisfied with a physician’s services or if you believe they did not fulfil their professional obligations, you can inform the CMQ by making a report. To do this, you have two options.

• Fill out the “Report concerning a physician” form; or
• Write a letter providing all the required information.
